DOES HERITAGE PARK OF KATY ACCEPT RESIDENTS WHO NEED TO APPLY FOR TEXAS MEDICAID BENEFITS?
Yes. If your loved one needs assistance to apply for Medicaid benefits while living in our facility, our Business Office Manager can assist you with this process.
How do i schedule a tour?
How do I get my loved one admitted?
Every case is different, but the process is the same, clinical and financial approval. Clinical: we need to make sure we are the right level of care for you loved one. Medication list, list of diagnose and a History and Physical are a great start, we will let you know if we any more medical records. Financial: We accept Medicare, Medicare Advantage, Medicaid, Medicaid Pending and Private Pay. Feel free to call our admissions office to discuss your case
Simply show up between 8am and 5:30pm and we will have someone to give you a tour and answer any questions you may have
​What transportation services are available?
Medical Transportation related to offsite doctor’s appointments are provided.
What are the visiting hours?
Visitation is from 7am to 9pm
Do you provide activities?
Yes, we have daily activities, including but not limited to: Bingo, board games, tv shows, ice cream socials, bible studies, movies and tv shows, pet therapy and many more
Do you provide Hospice Care?
Yes, Hospice Care is provided with the Hospice company of your choice
Is WiFi Accessible throughout Heritage Park?
WiFi is available throughout the facility for all our residents and visitors
Where do I send my loved one’s referral?
Most local Hospital sent them via different portals, we can receive them via email: admissions@hpkaty.com, Fax 281.395.1134, or simply bring it in and we will make a copy of everything
Is this facility 24/7?
Yes, our nursing care is round the clock care
